Ashtabula County, Ohio
In one of Ohio’s poorest counties, the city school district saved about $2.4 million in its first year with direct primary care, nurse navigators, a local pharmacy partner and direct specialist contracts. The city saved about $360,000.

Employers Health Cooperative of Wisconsin
Started by the Rice Lake school district after a $1,500 deductible led an employee to avoid care. Grew from about 750 covered lives in 2021 to about 18,600 in 2026, with free primary care at its core.

Southcentral Foundation’s customer-owned system reported 50% fewer emergency room visits, 56% fewer hospital admissions and 65% less specialist use.

Before your next renewal, ask your broker: Do you receive any commissions, bonuses or incentives from the carriers or vendors you recommend? Will you disclose all of your compensation in writing? Will we get full reports on our costs? Do we own our claims data?
